New Patient Form
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
On your first visit to our office, please arrive about 10 minutes early so that you have enough time to fill out our New Patient and Dental Health History forms.
Your first appointment will be approximately 90 minutes in duration. At the beginning of your appointment, your dentist will talk to you about your oral health needs in reference to your Health History form. Based on this conversation and a thorough dental exam, your dentist will develop a long-term dental treatment plan for you.
Your comprehensive examination will include an oral cancer screening, a full mouth series of x-rays, photos, and a thorough exam of your gums, teeth, bite and jaw.
Throughout your appointment, your dentist or hygienist will take the time to answer your questions about your dental health and treatment options.
To book an appointment with us, please call our office during our regular business hours. If you like, you can also book your next appointment while you're here at the office.
If you experience dental emergency, please call us right away and we'll make room in our schedule to see you.
To cancel or reschedule an appointment, please call us or visit us in person, rather than emailing.
Please let us know at least two business days before the date of your scheduled appointment, so we can open up the space in our schedule for another patient.
In the days before your appointment, you will receive automated appointment reminders from us by either text or email. If you must cancel or reschedule an appointment, please call us as soon as possible to let us know.
At Academy Dental Group, we accept most insurance plans. On your first appointment, please bring your insurance information with you, and we will be happy to submit your claim to your insurance company directly.
